Benefits
What does a health plan actually cover?
Coverage is more than a list of benefit categories. The plan documents, network, formulary, medical-necessity rules and cost sharing determine how a service is handled.

ACA Marketplace coverage
The ten essential health benefit categories
HealthCare.gov lists ten categories that Marketplace plans cover. Specific services can vary by state and plan, so a category name is not a promise of identical treatment or zero cost.
| Benefit category | What to look up in your plan |
|---|---|
| Ambulatory patient services | Outpatient care without hospital admission |
| Emergency services | Care for emergency situations under the plan rules |
| Hospitalization | Services such as surgery and overnight care |
| Pregnancy, maternity and newborn care | Care before, during and after birth |
| Mental health and substance use services | Including behavioral health treatment and counseling |
| Prescription drugs | The exact medications and tiers appear in the formulary |
| Rehabilitative and habilitative services | Services and devices for recovery or maintaining skills |
| Laboratory services | Covered tests under the plan's rules |
| Preventive, wellness and chronic disease services | Specific preventive rules and eligibility can matter |
| Pediatric services | Including oral and vision care requirements |
Coverage and cost are separate questions
A service can be covered and still require a deductible, copay or coinsurance. Some preventive services have special rules when delivered in-network, while the same service in another setting may be processed differently. Check both coverage and your share of the cost.
| Question | Where to verify it |
|---|---|
| Is the service covered? | SBC and full plan documents |
| Is the provider in-network? | Plan directory and the provider's office |
| Is the drug covered, and on what tier? | Current formulary |
| What will I pay? | Deductible, copay, coinsurance and out-of-pocket maximum |
| Are there limits or authorization rules? | Full plan documents and the plan's member services |
Puerto Rico note
Do not assume state and territory rules are identical
Medicaid in Puerto Rico has territory-specific funding, eligibility and administration rules. For Plan Vital, use current Puerto Rico Department of Health and Medicaid Program materials to confirm eligibility, benefits and renewal steps. Mainland Marketplace summaries are not a substitute.
Common questions
Does an essential benefit mean the service is free?+
No. Essential benefit categories describe coverage requirements, not universal zero cost sharing. Your plan's network and deductible, copay and coinsurance rules still matter.
Where can I find the drugs a plan covers?+
Check the plan's current formulary. Look for the exact drug, dosage form, tier and any authorization or quantity rules.
Is a provider directory enough to confirm network status?+
It is a starting point. Also contact the provider's office with the exact plan name and network, because directories and billing arrangements can change.
Are all health plans ACA Marketplace plans?+
No. Employer plans, Medicaid, CHIP, short-term products and other coverage have different rules. Identify the coverage type before applying Marketplace statements.
